As of July 1, 2018, New York State is changing the mandated health examination requirements. Physicals are now required for kindergarten and grades 1, 3, 5, 7, 9 and 11. Physicals must be submitted/mailed to the nurses’ office within the first 30 days of school. Proof of doctor appointment for mandated physical years will be accepted within the 30 day period.
If your child’s physical is less than a year old as of September 4, 2019 (the first day of school), you do not need another physical. Any further questions, contact my office at (631) 382-2104. The office is open during the summer months from 8:00 am – 3:00 pm.
All students who require medications during school hours must have a medication form filled out by their physician and signed by the parent/guardian. All medication (even over the counter medication such as Tylenol or Advil) must be brought into the Health Office by the parent/guardian in the original container with the student’s name clearly labeled on the container. Students new to Smithtown Schools
All students new to Smithtown Schools need to provide an Immunization Record from their doctor in order to register for school and provide the school they are attending with a recent physical. If no physical is provided the school doctor will perform a physical in the Health Office. All new students will receive vision, hearing and scoliosis screening according to NYS guidelines.
All students entering Kindergarten, grade 1, grade 3 and grade 5 are required to submit a recent physical to the Health Office. If no physical is handed in the student will receive a physical exam by a school doctor. All students will receive vision, hearing and scoliosis screenings as per NYS guidelines. All physical education excuses and absence notes go to the Health/Attendance Office. Any student who requires the use of crutches or Special Transportation needs their doctor’s note to state the accommodation and a time frame the student will need the accommodation (i.e. two weeks).
Middle School Students
All students entering grade 6 must provide proof of varicella (chicken pox) disease or immunization from their physician. Sixth grade students also must provide proof of TDAP vaccine by the time they reach their 11th birthday. All students in grade 7 MUST submit a physical exam to the Health Office or the school doctor will perform the exam in the Health Office. Students who wish to participate in any sport MUST have a physical by their doctor and it must be documented on the school district forms. All students will receive hearing, vision and scoliosis screenings as per NYS guidelines. All physical education excuses and absence notes are to be turned into the Health/Attendance Office. Students requiring Special Transportation, crutches, 5 minute/elevator/escort MUST have the doctor’s note state so and include a time frame for any accommodation (i.e. for two weeks).
High School Students
All students entering grade 9 and grade 11 must submit a physical exam to the Health Office or the school doctor will perform the exam in the Health Office. Students who wish to participate in sports MUST have a physical by their doctor and it must be documented on the school district forms. All students will receive hearing, vision and scoliosis screenings as per NYS guidelines. All physical education excuses must be turned into the Health Office. Any student requiring crutches, Special Transportation, 5 minute early pass /elevator/escort MUST have their doctor’s note state so and include a time frame for any accommodation (i.e. for two weeks). Absence notes are to be turned into the High School Attendance Office.
